Fees and Services
$3000.00 is the standard Global Fee for services.
Cash Pay Discounts:
• 10% discount from the Global Fee for paying entire fee at first visit
OR
• $200.00 discounted from the Global Fee for cash pay clients payed by the 37th week of pregnancy
- I offer a $50.00 referral bonus paid after new client signs on for care with Tree of Life Midwifery Care.
Non Refundable Deposit:
A non-refundable deposit is required at the first prenatal visit. The amount paid will vary depending on which Trimester of pregnancy that you are in at
the start of care. This one time, non-refundable deposit will be deducted from the Global Fee and the remaining amount due will be broken down into the
remaining payments.
• $400.00 - 1st Trimester (up to 12 weeks pregnant)
• $500.00 – 2nd Trimester (13-27 weeks pregnant)
• $600.00 – 3rd Trimester (28+ weeks pregnant)
Services Provided:
I understand that this fee INCLUDES:
• Prenatal, Labor/Delivery and Postpartum/Newborn Care
• Routine OB Profile blood work/Urine culture for mom
• Counseling regarding nutrition, pregnancy and breastfeeding
• Midwife services at birth
• Newborn exam with treatment of baby’s eyes with Erythromycin as is required by Texas Law
• Vitamin K administration to newborn, if necessary
• Newborn Screening at or before 36 hours and again at 2 weeks of age as required by Texas Law
• Filing of the birth certificate
• 24-36 hour postpartum visit and 2 week postpartum visit
• Well Woman Check and PAP Test at 8 weeks postpartum
I understand that this fee DOES NOT INCLUDE:
• Personal items for the birth (towels, baby clothes, sheets, toiletries, etc.)
• Birth Kit (supplies the midwife needs to do a home delivery – will discuss at the 35th week visit)
• Sonograms
• Lab work that is not routine
• Visits to a doctor or hospital as deemed necessary
• Rhogam or Winrho (for Rh- moms)
• If Vitamin K is desired for the newborn, a prescription must be obtained from a doctor and paid for by the client
• Fee for use of a birth center
• Ambulance fee if needed
• $250.00 assistance fee that is to be paid directly to the assistant after the birth ____
